For incoming Grade 7 FAPE applicants, the following must be submitted at the Principal’s office:
1. Original or colored photocopy of PSA birth certificate
2. One (1) pc. of 2×2 ID picture
3. Income Tax Return (ITR), Certificate of Tax Exemption (CTE) or Certificate of Unemployment
4. Photocopy of Grade 6 report card
